Albuterol
A familiar rescue medicine, with research specific to the condition, formulation and route.
One medicine, two names
Albuterol is called salbutamol in many countries. It relaxes the muscle around narrowed airways. Inhalers, nebulized solutions and oral tablets deliver the medicine differently. Levalbuterol is a separate medicine made from one of its two mirror-image forms.726,727,728
Products and strength conventions
The US inhalation-aerosol label covers bronchospasm and exercise-induced bronchospasm from age four. Other formulations have different age ranges. Its 108 micrograms of sulfate per actuation are equivalent to 90 micrograms of albuterol base. Product names and strength conventions vary by country.728,730,731
Changes in asthma care
Adding an anti-inflammatory medicine
MANDALA randomized 3,132 people with uncontrolled moderate-to-severe asthma. Participants continued their inhaled steroid-containing maintenance treatment. The higher-strength albuterol/budesonide rescue combination reduced the risk of a first severe exacerbation by 26% compared with albuterol alone.732

Adults with at least one severe exacerbation (%). FDA adult analysis: 71 of 1,180 with albuterol/budesonide and 126 of 1,173 with albuterol. Participants had uncontrolled mild asthma; follow-up lasted 12 to 52 weeks. Percentages are those reported in the label.733
Why the numbers differ across reports
BATURA randomized 2,516 people. Its paper analyzed 2,421 participants aged 12 and older, while the FDA label presents a 2,353-person adult analysis. The approved US combination remains for adults aged 18 and older. Each result belongs to its stated analysis group.734,733
Frequent rescue use
Large observational studies link frequent SABA use with more asthma exacerbations. People with worse asthma also use more rescue medication, which complicates cause and effect. The practical issue is persistent symptoms and whether the treatment plan is controlling them.735,729
Breathing and exercise
Exercise-induced bronchoconstriction
In the label study, 20 of 24 participants met the exercise-protection criterion after albuterol, compared with 6 of 24 after placebo. A separate study in 36 healthy adults found better breathing-test results after inhalation but no improvement in peak oxygen uptake.728,736
Regular use
A 255-person trial in mild stable asthma found little difference between scheduled and as-needed albuterol over 16 weeks. In a much smaller exercise-challenge study, one week of regular use reduced protection against exercise-induced airway narrowing. These studies measured different outcomes.737,738
COPD
An 85-day trial in 534 people with stable COPD found greater bronchodilation with ipratropium plus albuterol than with either medicine alone. Symptom scores were similar between groups. The combination result supports that particular COPD treatment comparison.739
Bronchiolitis in infants
A Cochrane review covering 30 trials and 1,992 infants found that bronchodilators did not improve oxygen saturation, reduce admission after outpatient treatment or shorten hospital stays. Tremor, faster heart rate and oxygen desaturation were reported.740
Muscle, fat and performance
Heart scans
Echocardiography found increases in wall thickness. Cardiac MRI found no between-group difference in heart structure or function, including left-ventricular mass. The small study was designed around lean mass and leaves long-term cardiac outcomes unresolved.741
One cohort, two papers
A second paper from the same training cohort reported a greater increase in mean sprint power, while peak power and strength improved similarly in both groups. These papers describe different measurements from the same participants.742,741
The inhaled study
A six-week study in 40 trained adults found a 0.8 kg greater fat-mass reduction in the female salbutamol group. It found no overall lean-mass advantage. Aerobic adaptation was slower early in training, and quadriceps strength was lower in the treated women. Results differed by sex and outcome.743
Side effects and the device
Shaking, heartbeat and potassium
Tremor, nervousness and a faster heartbeat are recognized effects. The inhaler label also covers low potassium, cardiovascular effects, allergic reactions and paradoxical bronchospasm, where breathing worsens after treatment. Worsening breathing or a rescue medicine that is no longer helping needs prompt medical assessment.728
Interactions
Beta blockers can oppose bronchodilation. Some diuretics can add to potassium loss; digoxin, MAO inhibitors and tricyclic antidepressants also appear in the interaction section of the label. A medication review should include the exact albuterol product and every other medicine used.728
An inhaler can spray after its medicine runs out
A 2025 Russian safety letter addressed a specific counterless, 200-dose Ventolin inhaler. It warned that the device may keep spraying after 200 actuations without delivering a salbutamol dose. Shaking or weighing it cannot reliably show how much medicine remains. Follow the instructions for the exact device.744
Device changes deserve attention
A German review of medication-error reports in 2026 described blocked valves, early emptying, missed sprays and technique problems after device switches. The device, remaining-dose indicator and inhalation technique can all affect delivery.745
Pregnancy
The label draws on observational studies and reports, which do not consistently show increased major birth-defect or miscarriage risk. It also describes the risks of poorly controlled asthma during pregnancy. Human milk concentrations were not measured in the evidence cited by this label.728
Evidence across countries
China: an inhaler comparison
A Chinese trial randomized 238 adults with asthma to a domestic salbutamol aerosol or an imported comparator; 224 entered the main analysis. Both improved lung function after one inhalation. The study measured a short-term breathing response and did not specify a formal noninferiority margin.746
China: the levalbuterol registry
The separate Chinese record CTR20240652 is a bioequivalence study of levalbuterol tartrate in 34 healthy participants. It was completed in May 2024, with no results summary posted in that record.747
Sport rules
Inhaled salbutamol has a conditional exception under the 2026 WADA List. Other routes and levalbuterol have different rules. Athletes need to check the exact product and applicable therapeutic-use-exemption requirements. Urgent treatment takes priority.748,749,750
Evidence gaps
The oral muscle studies are small, and long-term outcomes after high systemic exposure are not known; several regional databases could not be fully searched. This chapter uses the studies and official records identified in its references.741

01 What side effects should I watch for?
In the current US inhaler's six-week trial, headache was reported by 7% with albuterol and 2% with placebo, while tachycardia was reported by 3% and 0%; the table counted events whether or not investigators considered them drug related. Product labels also warn about paradoxical bronchospasm, fast or irregular heartbeat, low potassium, hypersensitivity, and harm from excessive use. Oral tablets and nebulizer solutions have separate adverse-event tables and populations.728,751,730
02 What conditions does albuterol treat, including asthma and COPD?
The current US inhaler is labeled from age four to treat or prevent bronchospasm in reversible obstructive airway disease and to prevent exercise-induced bronchospasm. In a 12-week trial of 534 adults with stable COPD, an albuterol-only arm improved lung function, while ipratropium-albuterol produced greater bronchodilation than either component. Asthma treatment also needs an anti-inflammatory controller plan when indicated.728,739,729
03 Can I use albuterol every day, and when is use too frequent?
Daily use can occur in a clinician-directed plan, but frequent rescue use is a warning signal. Current UK guidance calls for review when a SABA is needed more than twice weekly, and the US label calls for reevaluation when relief weakens or use increases. Scheduled-use trials found no routine advantage, and small EIB studies found reduced bronchoprotection with repeated use.729,728,737,738,752
04 How do a nebulizer, inhaler and spacer differ?
A nebulizer turns liquid medicine into a mist; a metered-dose inhaler delivers measured puffs, and a spacer helps the inhaled dose reach the lungs. In a 64-child acute-wheeze trial, an inhaler plus spacer and a nebulizer produced equivalent clinical improvement, with 3 of 32 children hospitalized in each group; 94% of parents found the spacer easier. Device, formulation, and dose are not interchangeable.753,728,730,745
05 How quickly does albuterol work, and how long does it last?
In a crossover EIB study, inhaled albuterol protected within 15 minutes but was no longer clinically effective at the four-hour exercise challenge; 20 people enrolled and 17 completed. The current inhaler label reports an adult terminal plasma half-life of about six hours after a high cumulative inhaled exposure. Blood half-life and felt bronchodilator duration are different measurements.754,728
06 Is albuterol a rescue medicine, and why is SABA-only asthma care changing?
Albuterol is a fast rescue bronchodilator. Current UK guidance no longer recommends SABA-only asthma care without an inhaled corticosteroid. In BATURA, the albuterol-budesonide combination reduced severe exacerbations compared with albuterol alone in uncontrolled mild asthma, so that result belongs to the combination product.729,734,733
07 Are albuterol, salbutamol and Ventolin the same medicine?
Albuterol is the US name and salbutamol is the international name for the same racemic active base. Ventolin is a product name; the sulfate salt, device and strength describe the specific formulation. Levalbuterol is the R-enantiomer and remains a separate medicine.755,756,726,727
08 Does albuterol burn fat or build muscle?
In a resistance-training trial, 30 men were randomized and 26 completed; oral salbutamol added 1.8 kg of lean mass versus placebo over 11 weeks, while fat mass did not clearly change and oxidative training adaptation worsened. Two papers report that same cohort. A separate 40-person inhaled study found a 0.8 kg fat-mass difference only in its 19-woman subgroup; there was no overall lean-mass treatment effect, but a multivariate analysis found female fat-to-lean repartitioning.741,742,743
09 Which albuterol interactions matter?
Beta-blockers can blunt bronchodilation and provoke bronchospasm. Non-potassium-sparing diuretics can worsen potassium or ECG effects, monoamine oxidase inhibitors and tricyclic antidepressants can amplify cardiovascular effects, and digoxin concentrations can change. The current label gives the product-specific interaction details.728
10 Does albuterol help exercise-induced bronchoconstriction?
Yes. In the current label's 24-person crossover exercise challenge, 20 of 24 participants were protected for the first hour after exercise versus 6 of 24 with placebo. A separate 24-adult trial found similar exercise protection with HFA and older CFC inhalers after one pre-exercise dose.728,757
11 How does albuterol open the airways?
Albuterol activates beta2 receptors on airway smooth muscle. That raises cyclic AMP, lowers intracellular calcium signaling, and relaxes narrowed airways. Beta selectivity is incomplete, which helps explain heart-rate and potassium effects.728
12 Are generic albuterol products available, and what affects cost?
FDA records include multiple approved albuterol applications, and FDA listed a first generic Ventolin HFA reference product on April 22, 2026. Cost varies with the product, device, strength, insurance, pharmacy, and supply. Discontinued, tentative, and combination application rows are not the same as a currently available single-ingredient inhaler.758,759
13 Can albuterol stop working or cause tolerance?
Yes, regular exposure can reduce bronchoprotection. In 13 people with mild asthma, protection fell after repeated doses and was significantly below its peak after seven doses; a separate 10-person EIB study found worse baseline and post-exercise lung function after one week of scheduled use. Larger mild-asthma trials found no exacerbation disadvantage but no routine benefit from scheduled albuterol.752,738,737,760
14 What happens when albuterol is stopped?
The bronchodilator effect wears off after albuterol is stopped, so breathing symptoms can return if the underlying airway disease is still active. The US label says weaker relief, worsening symptoms, or a need for more frequent doses should trigger prompt reassessment. Current asthma guidance treats frequent rescue use as a sign to review controller care.728,729
15 Does albuterol improve endurance, sprinting or gym performance?
In 36 healthy adults, acute inhaled salbutamol improved spirometry but not peak oxygen uptake or peak workload. A six-week inhaled trial in 16 male athletes found no added endurance, strength, or power benefit. Small oral studies reported sprint-power or torque gains at higher systemic exposure, with adverse effects and no consistent endurance benefit.736,761,762,763
