Where it stands

China approved mazdutide for weight management in June 2025 and adult type 2 diabetes in September 2025. The two prescribing documents cover different populations and clinical studies.1014

The glucagon question

Mazdutide activates two receptors. Animal experiments help explain its design; human trials measure the effects of the whole medicine. Its glucagon contribution to human calorie burning remains unsettled.1033,1015

Common questions

01 Has mazdutide actually been compared with semaglutide?

Yes. DREAMS-3 randomized 349 Chinese adults with early type 2 diabetes and obesity to mazdutide 6 mg or semaglutide 1 mg weekly. Its 2026 conference abstract reports better achievement of the combined blood-sugar and weight-loss target with mazdutide at 32 weeks. The comparison used the diabetes dose of semaglutide.1018,1019,1020

02 Was that a comparison with Wegovy 2.4 mg?

No. DREAMS-3 used semaglutide 1 mg. A separate trial, GLORY-3, compares mazdutide with semaglutide in people with excess weight and fatty liver disease. Its registry has no posted results, and its semaglutide dose description needs clarification.1018,1021

03 How did mazdutide compare with dulaglutide?

DREAMS-2 enrolled 731 Chinese adults with type 2 diabetes. At 28 weeks, mazdutide 4 mg and 6 mg reduced HbA1c by an additional 0.24 and 0.30 percentage points compared with dulaglutide 1.5 mg. Weight reductions were also greater, while gastrointestinal side effects were more common.1022

04 Can I stack mazdutide with semaglutide or tirzepatide?

The comparison trials assigned these medicines to separate groups. They do not provide a tested combination regimen. The cited evidence has not established the benefits or safety of taking mazdutide with semaglutide or tirzepatide.1018,1022

05 Is mazdutide better than retatrutide?

A direct answer needs a trial comparing them. Network analyses compare results from separate studies, which used different doses, durations and participants. Those rankings are useful research context, but cannot predict which medicine would work better for one person.1023,1024

06 Has mazdutide been tested outside China?

Yes. A phase 2 trial enrolled 179 adults at 24 US centres. It tested mazdutide against placebo over 48 weeks in people with obesity or overweight who did not have type 2 diabetes.1017,1025

07 Did weight loss continue after week 32 in the US trial?

The posted registry results show greater average reductions at week 48. The model-based estimates were 10.54%, 19.2% and 22.3% in the 3/6 mg, 10 mg and 16 mg groups, compared with 0.047% with placebo. These are estimates for the trial groups; the primary endpoint was at week 32.1025

08 What does the 20% dropout figure mean?

In the US phase 2 trial, 20% of participants assigned to 16 mg stopped treatment because of adverse events, mainly gastrointestinal problems. It refers to that dose group and that reason for stopping. It is not the dropout rate for every mazdutide study.1017

09 Why did the small early trial report about 20% weight loss so quickly?

That phase 1 study included just 32 people and tested escalation to 16 mg over 20 weeks. Its two active groups lost 20% and 21% on average. The later, larger US phase 2 trial provides a better view of how efficacy and tolerability vary across doses.1026,1017

10 How much weight did people lose in GLORY-1?

In 610 Chinese adults, average weight change at 48 weeks was -11.00% with 4 mg, -14.01% with 6 mg and +0.30% with placebo under the treatment-policy analysis. That analysis accounts for the assigned treatment even when someone stops it.1015

11 Did everyone lose at least 15% of their weight?

No. At 48 weeks in GLORY-1, 35.7% of the 4 mg group and 49.5% of the 6 mg group reached at least 15% weight loss, compared with 2.0% on placebo. The average and the share reaching a target are distinct measures.1015

12 Why do I see both 16.65% and 20.1% for GLORY-2?

The published JAMA abstract gives a 16.65% average reduction for the overall mazdutide group under its treatment-policy analysis. The conference abstract reports 20.1% for participants without type 2 diabetes, alongside 18.5% for the overall group in that presentation. Both the population and the analysis need to travel with the number.1016,1027

13 Did GLORY-2 include people with diabetes?

Yes. The published report says 16.1% of the 461 treated participants had type 2 diabetes. GLORY-2 included people with and without diabetes.1016,1027

14 Why do reports say either 461 or 462 participants?

GLORY-2 randomized 462 people, and 461 received treatment. The treatment groups contained 307 mazdutide recipients and 154 placebo recipients. Keep randomized and treated counts separate when comparing the reports.1016

15 Was the 9 mg trial easy to tolerate?

Stomach and bowel symptoms were common. In GLORY-2, vomiting occurred in 53.1% of mazdutide recipients, nausea in 46.9% and diarrhea in 39.4%. Most events were mild or moderate, and 2.9% stopped treatment because of adverse events.1016

16 Is mazdutide approved in China for both weight and blood sugar?

Yes. The Chinese manufacturer-hosted labels give approval dates of June 24, 2025 for weight management and September 16, 2025 for adult type 2 diabetes. Each indication has its own population and clinical data.1014

17 Does Chinese approval cover children?

The Chinese labels cover adults and say safety and effectiveness below age 18 have not been established. A 2026 conference abstract reports a 12-week randomized trial in 36 adolescents. It reported no severe or serious adverse events in the mazdutide groups. A larger adolescent phase 3 study is underway.1014,1028,1029,1030

18 Are there results for alcohol use disorder?

The registry lists a completed 308-person phase 2 study, but it had no posted results when checked. Its completion date establishes only the study status.1031

19 Is mazdutide the same kind of drug as tirzepatide?

They have different receptor targets. Mazdutide activates GLP-1 and glucagon receptors. Tirzepatide activates GLP-1 and GIP receptors. Results from one molecule should be checked against the trials of the other.1032,1014

20 Does the glucagon part make people burn more calories?

Researchers measured increased energy expenditure in mice given LY3305677, mazdutide’s development code. Human weight-loss trials measure the combined effect of the medicine. How much extra calorie burning contributes in people remains unsettled.1033,1034,1015

21 Does mazdutide preserve muscle?

The weight results quoted here measure total body weight. They do not tell us how much of the change was muscle, other lean tissue or fat. A muscle-preservation claim needs body-composition measurements from the mazdutide study being cited.1015,1017

22 Can it improve fatty liver disease?

Human trials have reported reductions in liver fat measured by MRI, including an exploratory GLORY-1 analysis. The largest quoted reduction, 80.2%, came from the 6 mg group with at least 10% liver fat at baseline. Studies of biopsy-confirmed MASH and lasting liver outcomes are still needed to establish those benefits.1035,1036,1015

23 Does it protect memory or prevent dementia?

The published cognition experiment used male diabetic mice. A human trial called LIGHT-COG is recruiting people with type 2 diabetes and early cognitive impairment. Its registry has no posted results yet.1037,1038

24 How long does mazdutide stay in the body?

The Chinese weight-management label gives a mean elimination half-life of about 10 days. The diabetes label gives 196.3 hours. The first-in-human abstract reported geometric mean half-lives of 7.5 to 9.8 days. PK charts should identify the study and population behind their estimate.1014,1039,1040

25 Why is a weekly schedule studied when its half-life is longer?

The trials and Chinese labels use weekly treatment. Half-life describes how quickly the body clears a medicine; it does not determine a schedule by itself. The labels report that repeated weekly dosing reaches steady state after about four weeks.1014

26 What are the most common side effects?

Nausea, diarrhea and vomiting are common in the trials. Their frequency varies with the dose, population and study. GLORY-1, GLORY-2 and the higher-dose US trial should each keep their own adverse-event numbers.1015,1016,1017

27 Does the thyroid warning mean it caused thyroid cancer in people?

The Chinese label warning comes from thyroid C-cell tumors in rodents; its relevance to humans is uncertain. The label excludes people with a personal or family history of medullary thyroid carcinoma or MEN2.1014

28 Can it affect heart rate?

Yes. In the Chinese weight-management label, heart rate rose by an average of 2.6 beats per minute at week 48 in the pooled mazdutide group, with no increase in the placebo group. Small early high-dose cohorts reported larger increases.1014,1040

29 Can it cause low blood sugar?

Other medicines that lower blood sugar are part of the risk assessment. The Chinese label calls out insulin and insulin secretagogues such as sulfonylureas. Diabetes-trial safety results need to retain the background medicines participants were taking.1014,1041

30 Can it affect other tablets I take?

The Chinese label says delayed stomach emptying can affect absorption of oral medicines. It calls for close monitoring with medicines that have a narrow therapeutic range, such as warfarin. A medication review should include prescription drugs, supplements and the reason each is taken.1014

31 What does the label say about pregnancy?

The Chinese labels contraindicate use during pregnancy and specify stopping at least three months before a planned pregnancy. Discuss pregnancy plans with the prescriber early enough to arrange appropriate treatment.1014

32 What about breastfeeding?

The Chinese labels say mazdutide should not be used during breastfeeding. Whether it passes into human milk is unknown, and risk to the infant cannot be excluded.1014

33 Were older adults included?

The weight-management label reports only three participants aged 65 or older. The diabetes label reports 107 treated participants aged 65 or older across two phase 3 studies. Those are different evidence bases.1014

34 Can rapid blood-sugar improvement affect the eyes?

The diabetes label warns that rapid improvement in glucose control can temporarily worsen diabetic retinopathy. It calls for monitoring people with a history of retinopathy; patients needing urgent treatment for advanced eye disease were not studied.1014

35 Can a purity claim prove what is in a research vial?

A claim needs a report that identifies the material and the methods used. The analytical-method paper includes mazdutide in method development, but its reported commercial-product testing covered other medicines. It cannot verify a separate mazdutide vial.1042

36 Are there signs of kidney benefit?

A conference analysis of the 179-person US trial reported improvements in cystatin-C-based filtration estimates and urine albumin at 32 weeks. These are kidney markers. Whether mazdutide prevents kidney failure requires longer outcome trials.1043,1025

37 Does an 80.2% liver-fat reduction mean losing 80.2% of body fat?

It refers to the relative change in fat measured inside the liver by MRI. The result came from a GLORY-1 subgroup whose liver fat was at least 10% at baseline, after 48 weeks in the 6 mg arm. Total body fat and body weight are different measurements.1035

38 Could mazdutide help with uric acid or gout?

Trials have reported lower blood uric acid, and rat studies have explored changes in purine and fat metabolism. Researchers have also registered studies in hyperuricemia and gout. The cited sources do not establish prevention of gout attacks.1044,1045,1046,1053

39 Does a research-use label settle whether a product can be sold as a medicine?

FDA warning letters have addressed mazdutide products promoted for human use despite research-use labeling. The agency assessed the marketing and intended use. Those letters also give a concrete record of US enforcement around unapproved mazdutide products.1047,1048

40 Do more reviews mean more people have been studied?

Several reviews can reuse the same trials. Conference abstracts, journal papers and registry entries can also describe the same participants. This guide keeps those links visible so a new publication does not inflate the trial population.1049,1050,1051

41 Does mazdutide have an approved safety label?

Yes. China has approved prescribing information for weight management and type 2 diabetes. It includes contraindications, warnings and adverse-reaction tables. Those are the relevant documents when checking claims that no formal safety information exists.1014

42 Has hair loss been reported?

Yes. The Chinese weight-management label reports hair loss in 1.2% of mazdutide recipients and 0.5% of placebo recipients in GLORY-1. The reported cases were mild.1014

43 Is mazdutide banned in sport because it lacks FDA approval?

FDA status alone cannot answer that. WADA’s S0 wording considers approval by any governmental health regulator, and mazdutide has Chinese approval. This review has not established its overall sport eligibility; athletes need a current check with their governing anti-doping organization.1052,1014
