A specific neurological use

The UK Nootropil label covers add-on treatment of cortical myoclonus in adults. A small crossover trial found improvement in 21 people selected because they appeared to respond to piracetam. Ten needed rescue treatment during placebo periods.^1227,1228

Memory in adults

A 2024 review combined 18 studies involving 886 adults with memory impairment. The pooled effect was uncertain: standardized mean difference 0.75, 95% CI -0.19 to 1.69. Results varied substantially between studies.¹²²⁹

Memory and stroke

Healthy people

Small, older experiments reported selective changes in word learning or performance under low oxygen. A separate study found EEG changes without better neuropsychological test performance. Evidence for everyday study and work benefits is uncertain.^{1230,1231,1232,1233}

Dementia

The later Cochrane review body listed 24 studies, but only eight with 526 participants supplied usable data. It found no clear improvement in specific cognition, dependency or depression measures. A favorable global rating came from four studies analyzed among completers.¹²³⁴

PASS found no overall benefit on its main outcome. Mortality was 111/464 with piracetam and 89/463 with placebo; this difference was uncertain. A later trial, PASS II, stopped after 571 participants following an interim analysis.^1235,1236

Language after stroke

A review of seven trials involving 261 participants found no clear overall aphasia benefit, with a small signal on written language. The 2016 Korean rehabilitation guideline advised caution about routine use.^1237,1238

Other clinical questions

After heart surgery

Some small bypass-surgery studies reported cognitive benefits. In a 98-person trial, the result was positive among participants who followed the protocol but uncertain when analyzed by assigned group. An 88-person valve-surgery study found no benefit.^{1239,1240,1241}

Reading difficulties

Older dyslexia trials reported mixed results. One independent trial enrolled 60 children and found no overall reading benefit. A 2016 German guideline report recommended against piracetam for reading and spelling disorders.^1242,1243

5 trials

A 2024 review of five trials involving 437 children reported better responses with piracetam. Results varied between measures and studies, and the adverse-event comparison was too imprecise to settle safety.¹²⁴⁴

Regional research and mechanism

Chinese combination study

A 2000 trial enrolled 496 people with age-associated memory impairment and tested choline plus piracetam. Its abstract reported favorable changes, but the printed mean changes do not match subtraction of its before-and-after scores. The mixture leaves each ingredient’s contribution unresolved.¹²⁴⁵

Russian reports

A 60-person study compared two doses without placebo. A larger report followed 1,183 people without a control group. Both reported improvement, with unresolved denominator inconsistencies that weaken confidence in their estimates.^1246,1247

A newer formulation study

A 2026 Chinese study tested bioequivalence in 56 healthy volunteers. It compared how two tablet products entered the bloodstream. The manufacturer funded the work and employed three authors.¹²⁴⁸

Laboratory findings

Experiments explore weak AMPA-receptor binding, membrane properties and cellular responses to oxygen or metabolic stress. The models include isolated proteins, animal tissue and cultured cells. Their connection to clinical effects is uncertain.^{1249,1250,1251,1227}

Safety and product quality

Kidneys and other medicines

Piracetam is largely excreted unchanged in urine. The UK label calls for renal dose adjustment and contraindicates use below a creatinine clearance of 20 mL/min. It also flags bleeding risk, anticoagulants, antiplatelet medicines and surgery.¹²²⁷

Who needs particular care

The UK label contraindicates use with cerebral hemorrhage or Huntington’s chorea. It advises against abrupt withdrawal in myoclonus because seizures may follow. Pregnancy and breastfeeding require an individual clinical assessment.¹²²⁷

Products sold as supplements

A 2018 purchase sample tested ten products from five brands. Eight samples from four brands contained piracetam, at 85% to 118% of their labeled amount. The study describes those purchases; it cannot verify a product sold today.¹²⁵²

01 Is piracetam a peptide?

No. Piracetam is a small molecule in the racetam family. Nootropil and Myocalm are product names associated with it.^1255,1256

02 Is piracetam approved in the United States?

Piracetam has no FDA-approved use. FDA has also excluded it from the 503A bulk-substance list used for certain pharmacy compounding.^1253,1254

03 What is piracetam prescribed for in the UK?

The current UK Nootropil label covers cortical myoclonus in adults, alongside other anti-myoclonic treatment. This is a movement disorder involving brief muscle jerks.¹²²⁷

04 What did the myoclonus trials find?

A small crossover trial in 21 people who appeared to respond to piracetam found less myoclonus on treatment. Ten people needed rescue when assigned placebo. Selecting apparent responders before the comparison makes the result most relevant to that group.¹²²⁸

05 Will piracetam help me study or work?

In a study of 12 volunteers exposed to simulated altitude, piracetam reduced errors on one task without improving processing speed. An oxygen-deprivation experiment offers limited guidance about ordinary studying or work.¹²³²

08 Did the large mild cognitive impairment trial work?

No. The 676-person N01001 trial found no significant advantage on its main cognitive outcome at either studied dose after 12 months. The sponsor report contains the results; the old registry record has no posted result tables.^1225,1226

09 Does piracetam treat dementia?

The updated Cochrane review body found no significant benefit on specific cognitive measures, dependency or depression. Its favorable global-impression result came from participants who completed the studies and was inconsistent between studies, so the review does not establish a useful dementia treatment.¹²³⁴

10 Why do some summaries sound more positive?

Some emphasize a global clinical rating or a favorable subgroup while giving less space to negative cognitive tests, incomplete follow-up and unpublished data. Reading the main outcome and the participant flow gives a clearer picture.^1234,1225

11 Did piracetam improve recovery after an acute stroke?

The 927-person PASS trial found no overall benefit on its main outcome. A favorable subgroup identified after the main analysis is a lead for further research, rather than a confirmed treatment effect.¹²³⁵

12 What happened to PASS II?

The registry lists PASS II as terminated after enrolling 571 people. It says an interim analysis found a less than 20% chance of demonstrating the planned treatment difference. No arm-level results are posted in that registry record.¹²³⁶

13 Did the stroke review find a safety problem?

The review found an uncertain increase in early death, with a confidence interval spanning no difference. It also found insufficient evidence of better recovery. Sparse adverse-event reporting limits the safety assessment.¹²⁵⁷

14 Does piracetam protect memory during heart surgery?

Some small bypass-surgery trials reported better short-term test scores. Results were mixed: in a 98-person trial, the analysis of participants who followed the protocol was positive, but the analysis by assigned treatment group was not statistically significant.^{1239,1258,1240}

17 Does it help children with dyslexia?

Older trials reported some gains in reading speed, with less consistent effects on other skills. A separate 60-child trial found no overall reading benefit. Possible overlap between several published cohorts makes adding their sample sizes misleading.^{1259,1260,1261,1242}

18 What did the Chinese memory study test?

It tested choline plus piracetam against placebo in 496 people with age-associated memory impairment. The abstract reports better memory scores with the combination. The full paper is inaccessible, the component ratio is unresolved, and some printed change scores do not match the printed before-and-after means.¹²⁴⁵

19 Does that study prove I need to add choline?

It cannot answer that question. It compared a combined product with placebo, without a piracetam-only group to show what adding choline changed.¹²⁴⁵

20 What do the Russian studies add?

They add clinical observations, including a large uncontrolled report in people with hypertension and cognitive symptoms. Weak controls, concurrent treatment and inconsistent denominators make the size of any piracetam effect uncertain.^{1246,1247,1262}

Piracetam is largely cleared through the kidneys. The UK label adjusts use according to renal function and contraindicates it in severe renal impairment below its stated creatinine-clearance threshold. Reduced clearance can increase exposure.¹²²⁷

24 Can piracetam affect bleeding?

The UK label warns about its effects on platelet aggregation. It calls for caution with bleeding disorders, gastrointestinal ulcers, major surgery and medicines that affect clotting, including anticoagulants and antiplatelet drugs.¹²²⁷

25 Can someone stop it suddenly?

Abrupt withdrawal can trigger myoclonic or generalized seizures in some people treated for myoclonus, according to the UK label.¹²²⁷

Adequate human pregnancy data are lacking. Piracetam crosses the placenta, and the UK label reports newborn concentrations at 70% to 90% of maternal concentrations.¹²²⁷

27 Do adverse-event database counts show how risky it is?

They identify reports worth examining, but cannot calculate someone’s chance of harm. Reports can involve several medicines, illness and duplicate information. The piracetam/nootropil query includes every listed drug role, including reports where piracetam was not considered the cause.¹²⁶³

28 Have products sold as piracetam matched their labels?

A laboratory study tested ten samples from five brands purchased in 2018. Four brands contained piracetam; one had no detectable amount. Among the positive products, measured amounts ranged from 85% to 118% of the label claim.¹²⁵²

30 Is new clinical research still happening?

An Iranian registry lists a recruiting 2026 trial of cognition in 90 older adults with depression. It has no posted results. Its dose and randomization fields contain conflicting descriptions, which need clarification before interpreting a future report.¹²⁶⁴

31 Are piracetam and levetiracetam interchangeable?

They are different medicines. The UK piracetam label concerns adjunctive treatment of cortical myoclonus; levetiracetam reports need their own drug-specific interpretation. A case report about one should not be cited as an adverse effect of the other.^{1227,1265,1266}

32 Can piracetam prevent another stroke?

A 563-person trial compared piracetam with aspirin for two years after stroke. 35 How might piracetam work?

The UK label says its clinical mechanism in cortical myoclonus is unknown. In an isolated AMPA-receptor crystal experiment using 5 mM piracetam, binding occurred at multiple weak, low-occupancy sites; another study found membrane-fluidity changes in aged animal tissue, ex vivo human-derived tissue, and treated rats.
