A blood-pressure medicine

Telmisartan blocks the angiotensin II receptor. It treats high blood pressure and has a cardiovascular-prevention indication for certain high-risk adults. The US indication covers people age 55 or older who cannot take ACE inhibitors. European eligibility is worded differently.^654,655

Labels differ by country

The usual US starting dose for hypertension is 40 mg by mouth once daily. The Canadian Micardis monograph recommends 80 mg once daily. These are labeled regimens; the prescription and leaflet for the actual product determine its use.^654,656

Major cardiovascular trials

ONTARGET: compared with ramipril

ONTARGET randomized 25,620 adults with vascular disease or high-risk diabetes after a three-week run-in. Over a median 56 months, the main cardiovascular outcome occurred in 16.7% taking telmisartan and 16.5% taking ramipril. Telmisartan met the trial's noninferiority criterion, with less cough and angioedema but more hypotensive symptoms. Combining the drugs increased adverse effects without improving that outcome.⁶⁵⁷

TRANSCEND: the primary result was neutral

Among 5,926 high-risk adults unable to tolerate ACE inhibitors, the main cardiovascular outcome occurred in 15.7% with telmisartan and 17.0% with placebo. The difference was not statistically significant. A narrower secondary outcome favored telmisartan before adjustment for multiple comparisons, but its adjusted p value was 0.068.⁶⁵⁸

PRoFESS: after ischemic stroke

PRoFESS enrolled 20,332 people after an ischemic stroke. Recurrent stroke occurred in 8.7% with telmisartan and 9.2% with placebo over a mean 2.5 years, a nonsignificant difference. Major cardiovascular events and new diabetes also showed no significant reduction.⁶⁵⁹

A newer result after brain hemorrhage

TRIDENT tested a three-drug pill

The 2026 trial studied 1,670 adults after intracerebral hemorrhage. After an active run-in, participants received either telmisartan, amlodipine and indapamide together or placebo, added to standard care. The studied daily pill contained 20 mg, 2.5 mg and 1.25 mg, respectively.⁶⁶⁰

More treatment discontinuation

Adverse events led to early discontinuation in 13.6% of the triple-pill group and 6.0% of the placebo group. A rise of at least 20% in creatinine was the most common reason. The benefit and harms belong to this combination and this stroke population.⁶⁶⁰

Insulin resistance and body fat

The mechanism attracted interest

Laboratory studies found partial PPAR-gamma activity, prompting trials of insulin resistance and body-fat changes. Human results vary with the population and outcome measured.^661,662

The larger insulin-resistance trials were negative

A 138-person trial in overweight or obese adults without hypertension or diabetes found no insulin-sensitivity benefit after 16 weeks. TAILoR recruited 377 people with HIV receiving antiretroviral therapy; telmisartan also missed its primary insulin-resistance outcome at 24 weeks. TAILoR reported marginal improvements in selected secondary measures.^662,663

Regional studies add useful detail

A Chinese abstract reports 120 randomized participants with diabetic kidney disease, with 112 remaining in follow-up. At six months, telmisartan added to routine care produced larger changes in urine protein, homocysteine, cholesterol and carotid intima-media thickness than routine care alone. A Korean pilot randomized 32 people with type 2 diabetes to telmisartan or valsartan for 12 weeks: neither treatment had a clear advantage, and glucose, insulin-resistance and lipid measures were unchanged.^664,665

Safety and interactions

Kidneys, potassium and blood pressure

Low blood pressure, high potassium and worsening kidney function are central risks. Dehydration, kidney disease, NSAIDs, potassium supplements and combining renin-angiotensin blockers can increase them. The label also flags lithium and digoxin interactions.⁶⁵⁴

Stomach symptoms and dizziness

Current European information includes intestinal angioedema, a swelling in the gut that can cause abdominal pain, nausea, vomiting and diarrhea. Its frequency for telmisartan is unknown. A separate European review supported adding dizziness to the product information.^655,666,667

Alcohol and sudden standing

The UK product information warns that alcohol may worsen the blood-pressure drop on standing. Tablets may be taken with or without food.⁶⁶⁸

Comparisons and product identity

The Russian comparison was observational

A Russian study followed 80 adults older than 60, with 20 receiving each of four blood-pressure treatments. Treatment was selected by physicians. The small, three-month comparison adds local clinical context; randomized trials provide the stronger comparison evidence.⁶⁶⁹

One ingredient, several kinds of product

Human telmisartan tablets, fixed combinations and veterinary Semintra share an ingredient but have different formulations and uses. Micardis HCT adds hydrochlorothiazide; Widaplik combines telmisartan, amlodipine and indapamide.^670,671

Sport rules by ingredient

Telmisartan was removed from WADA's Monitoring Program in 2018. The 2026 and 2027 lists name hydrochlorothiazide and indapamide as prohibited diuretics. Athletes need the status of their exact product, including combination ingredients.^672,673,674

01 When should telmisartan be taken, and how quickly does it work?

The US label says telmisartan is taken once daily and can be taken with or without food; it does not require morning or night dosing. Most blood-pressure lowering appears within two weeks, with the maximum response usually reached by four weeks.⁶⁵⁴

02 What side effects should I watch for?

In US hypertension trials, upper-respiratory infection occurred in 7% with telmisartan and 6% with placebo; back pain was 3% versus 1%, diarrhea 3% versus 2%, and sinusitis 3% versus 2%. More important label risks include low blood pressure, high potassium, kidney injury, fetal harm, and intestinal angioedema of unknown frequency; the 2025 EU safety review also supported adding dizziness.^654,655,667

03 What is telmisartan approved to treat?

Telmisartan is approved for hypertension. The US label also includes reduction of myocardial infarction, stroke, or cardiovascular death in high-risk adults age 55 or older who cannot take ACE inhibitors, while the EU prevention wording and population differ.^654,675,655

04 Which telmisartan interactions matter?

NSAIDs can blunt the blood-pressure effect and worsen kidney function; lithium levels can rise; and the label reports 49% higher peak and 20% higher trough digoxin concentrations. Combining renin-angiotensin system blockers raises the risk of low blood pressure, high potassium, and kidney injury, and aliskiren is contraindicated in people with diabetes.⁶⁵⁴

Micardis and approved generic telmisartan products share telmisartan as the active ingredient. Fixed combinations add other medicines and have their own product instructions.^676,654,670

06 How does telmisartan compare with losartan, olmesartan, valsartan, or ramipril?

ONTARGET found telmisartan noninferior to ramipril in high-risk patients, with less cough and angioedema but more hypotensive symptoms. Diabetic-kidney trials found greater proteinuria reduction than losartan but a similar reduction to valsartan; the Russian 80-person study was observational, leaving superiority to olmesartan open.^{657,677,678,669}

07 Can telmisartan raise potassium or affect kidney function?

Yes. The labels warn that potassium can rise and kidney function can worsen, especially with kidney disease, volume depletion, NSAIDs, potassium-raising medicines, or dual renin-angiotensin blockade; the ONTARGET combination arm had more renal dysfunction and hyperkalemia.^654,657

08 Does telmisartan improve insulin resistance or diabetes?

Results are mixed. Several small trials in hypertensive patients with metabolic syndrome reported better glucose or insulin-resistance measures, but a 138-person placebo-controlled study and the 377-person TAILoR trial missed their primary insulin-sensitivity outcomes.^{679,680,662,663}

09 Does telmisartan protect people using anabolic steroids?

The cited controlled studies involved hypertension, vascular disease, diabetes, stroke, kidney disease, or other clinical populations. None tested telmisartan as protection from the cardiovascular effects of anabolic-steroid use.^657,658,659

10 Does telmisartan cause weight loss or reduce visceral fat?

Telmisartan is not established as a weight-loss drug. A 53-person metabolic-syndrome study reported lower visceral fat, while a 138-person placebo-controlled study found no significant body-weight or fat-mass benefit.^681,662

11 What should I know about a missed dose or stopping telmisartan?

Missed-dose wording differs by label: the US leaflet says to take the dose when remembered unless it is close to the next dose, while the Canadian monograph says to take the next dose at the usual time. Neither says to double the dose. In US trials, blood pressure returned gradually to baseline over several days to one week after telmisartan stopped.^654,656

12 Can telmisartan be taken with food or alcohol?

US labeling allows telmisartan tablets with or without food. The UK product information says alcohol can aggravate the drop in blood pressure when standing, which can increase dizziness or fainting.^654,668

13 How long does telmisartan stay in the body?

Peak blood levels usually occur about 0.5 to 1 hour after a dose, and the terminal half-life is about 24 hours. Elimination is mainly through bile and feces as unchanged drug, while the main glucuronide metabolite is inactive.⁶⁵⁴

14 Does telmisartan improve exercise or athletic performance?

In 114 people with peripheral artery disease, six months of telmisartan did not improve six-minute walk distance or five secondary performance outcomes compared with placebo.⁶⁸²

15 Does telmisartan affect erections or libido?

In men at high cardiovascular risk, randomized telmisartan, ramipril, combination therapy, or placebo did not significantly improve or worsen erectile dysfunction in an ONTARGET and TRANSCEND substudy. The study did not report a libido outcome.⁶⁸³
