L-carnitine

L-carnitine helps cells use fat for energy. Its medical role centers on deficiency; weight-loss and performance studies give mixed results.

US oral Carnitor treats primary systemic carnitine deficiency and secondary deficiency caused by certain inherited metabolic disorders. The IV label also covers deficiency in patients with end-stage kidney disease receiving dialysis.867,868

A safety result to understand

A six-month trial randomized 177 adults with metabolic syndrome and analyzed complete scans from 157. Its main plaque-volume measure was unchanged, while an exploratory analysis found greater carotid narrowing with carnitine.869,870

Common questions

01 What is L-carnitine actually prescribed for?

US oral Carnitor treats primary systemic carnitine deficiency and secondary deficiency caused by certain inherited metabolic disorders. The IV label also covers deficiency in patients with end-stage kidney disease receiving dialysis.867,868

02 How much weight loss has it produced in studies?

The 2020 review estimated 1.21 kg less body weight than control across 24 trials involving 1,520 people. One included PCOS trial now carries an expression of concern. The estimate needs a fresh analysis that accounts for that problem.864,865,866

03 Why do some trials find no weight-loss benefit?

Results vary with the people, treatment and study design. In an eight-week trial of 36 moderately overweight women following the same walking program, L-carnitine produced no significant advantage in body weight or fat mass. Five women assigned carnitine withdrew because of nausea or diarrhea.871

04 Did the Chinese study show four kilos of fat loss?

It measured weight loss. Thirty adults with metabolic syndrome completed a short hospital program combining severe calorie restriction, walking and IV carnitine or saline. Weight fell by 4.6 kg with carnitine and 3.2 kg with saline; the study never measured how much of that change was fat.872

05 What is the concern about TMAO?

A 24-week study in healthy older women found a tenfold rise in this carnitine metabolite while the measured inflammatory and lipid markers stayed unchanged. The health effects of supplementation require evidence from clinical outcomes as well as blood tests.873

06 Has a trial raised concerns about artery narrowing?

Yes. A six-month trial randomized 177 adults with metabolic syndrome and analyzed complete scans from 157. Its main plaque-volume measure was unchanged, while an exploratory analysis found greater carotid narrowing with carnitine.869,870

07 Can L-carnitine trigger seizures?

The US labels include seizure reports, including worsening in some people who already had seizures. A 2016 review found no published study supporting seizure induction in patients taking valproate. The available reports do not establish how often a causal reaction occurs.868,867,874

08 Does it interact with warfarin?

Increased INR has been reported. The US label calls for INR monitoring when levocarnitine treatment starts or its dose changes.867

09 Why does kidney function matter?

The oral label warns that certain metabolites may accumulate during chronic high-dose use in severe kidney impairment because they normally leave through urine. Dialysis patients also have a distinct medical indication for IV treatment, which requires separate consideration.867,868

10 Does it reliably improve fatigue during dialysis?

The evidence is mixed. An older trial reported improvement in a fatigue score, but the later Cochrane review found little or no overall fatigue benefit across two studies involving 353 people. The certainty was low.875,876

11 Could it improve exercise performance?

A small study in 14 men reported improved performance after 24 weeks of carnitine plus carbohydrate. A 2025 crossover study in 20 trained men found no improvement from a single pre-exercise carnitine-tartrate treatment. The studies tested very different regimens.877,878

12 Are the oral and IV findings interchangeable?

Each study applies to the formulation and route it tested. The US labels have separate indications and absorption information for oral and IV products. Oral solutions are explicitly labeled for oral use only.867,868

13 Did the newer synbiotic study prove carnitine works?

Both groups received carnitine. The eight-week study in 46 women compared adding a synbiotic with adding a placebo, so the reported differences concern the synbiotic addition.879

14 What did the additional Chinese research find?

A 2004 study followed 16 women for 12 weeks and reported better weight and fat measures with carnitine plus exercise. Its Chinese and English abstracts disagree about the group labels and printed dose, leaving the exact result unresolved.880

15 Is there Russian research on memory?

A 2025 Russian study followed 104 adults with hypertension and cognitive difficulties through ten days of treatment. It reported improvement on some memory, attention and fatigue measures. The open design, brief follow-up and inconsistent statistical labeling limit the finding.881

16 Have any studies been retracted?

Yes. Several PCOS trials and a dialysis report have been retracted. A separate PCOS weight-loss trial has an expression of concern, and it was included in the widely cited 2020 weight review.882,883,884,885,886,866,864

17 Have injections actually been studied for fat loss?

A 2026 report followed five people through a program combining injected substances, ultrasound, electrical stimulation, diet and exercise. Body measurements improved, but the study could not separate carnitine from the other treatments.887

18 Do other countries prescribe it?

Japan has oral and IV levocarnitine products for carnitine deficiency. The UK authorized another generic IV product in April 2025. The indications depend on the product and country.888,889,890

19 Could an IV violate sports rules?

Yes. WADA restricts IV infusions and injections above a cumulative 100 mL in 12 hours, with specific medical exceptions. The rule can apply even when the substance itself is permitted.891,892

20 Did it improve quality of life in liver disease?

A Korean trial randomized 150 adults with cirrhosis and covert hepatic encephalopathy to carnitine or placebo for 24 weeks. Quality of life improved within the carnitine group, but the change was comparable between groups.893

21 Could it affect my thyroid medication?

L-carnitine can oppose some thyroid-hormone actions. In a small trial of people taking levothyroxine, mental fatigue improved while overall and physical-fatigue scores did not significantly change. Thyroid treatment belongs in the medication review before adding it.894,895

22 If it helps cells use fat, why might a supplement do nothing?

Healthy adults make carnitine themselves. EFSA found that its established role in fat metabolism was insufficient to support the proposed benefit from extra consumption in healthy people. Whether a supplement helps depends on the clinical evidence and the person’s condition.896

23 Does it help with PCOS?

One 62-person Iranian trial found improvements in insulin-resistance measures, with no significant benefit for blood glucose or the measured lipids. Another report from the same trial found no advantage in calculated liver-fat and cardiovascular-risk indices. These papers did not measure pregnancy or live-birth benefit.897,898,899

24 What did the Korean exercise study show?

An eight-week study of 18 women reported better fat-percentage and fat-free-mass measures with carnitine plus exercise. It found no improvement in fitness or metabolic-risk measures. The full tables are needed to check the size and precision of the body-composition result.900

25 Are acetyl-L-carnitine and L-carnitine the same thing?

They are different compounds. Studies may test either form, a carnitine salt, or a combination. Check the actual formulation before applying a study’s result to a product.901,902,903

26 Has it been studied for male fertility?

A Chinese trial compared combined L-carnitine and acetyl-L-carnitine with vitamins C and E. Among men who completed treatment, partners became pregnant in 10 of 85 cases versus 2 of 53. The combination study reported no live-birth result in its abstract.903

27 Did Canada ban L-carnitine?

Health Canada’s 2024 decision concerns adding it to supplemented foods. The agency found insufficient evidence to set acceptable conditions for that use. Medicine and natural-health-product authorizations are separate product categories.904

28 How long does L-carnitine stay in the body?

A small IV study in ten healthy people reported a terminal phase of 10 to 23 hours. The US injection label reports 17.4 hours under its specified study conditions. Those figures apply to the reported injection conditions. Oral absorption and other injection-route pharmacokinetics require separate evidence.905,868
