Dutasteride
Dutasteride lowers DHT by blocking two forms of an enzyme. Its main clinical evidence concerns an enlarged prostate and male-pattern hair loss.
An enlarged prostate
Across three US-label trials involving 4,325 men, urinary retention occurred in 1.8% with dutasteride and 4.2% with placebo over two years. Prostate-related surgery occurred in 2.2% and 4.1%, respectively.840
Country-specific approvals
The US Avodart label covers BPH. Japan’s Zagallo label includes male-pattern hair loss.840,841
Formulations
Topical research
A 135-man phase 2 trial reported improved hair counts with one specific solution. It detected dutasteride in blood and has unresolved discrepancies in participant totals and exposure reporting.842
Regional studies
Chinese BPH studies include uncontrolled add-on treatment and retrospective predictors of retention or surgery. A small Iranian oral-hair trial found no significant difference from finasteride at 24 weeks.845,846,847
Sexual effects, fertility and mood
Fertility measurements
Studies report changes in sperm count, semen volume and motility. The newer 200-man clinic study lacked pre-treatment semen measurements and an untreated comparison group.840,848
Updated mood warnings
Regulators added precautionary warnings after reviewing the drug class. The European review found insufficient evidence for a direct dutasteride link; UK advice calls for medical contact if depression or suicidal thoughts occur.849,850
Slow clearance
The US label gives a steady-state half-life of about five weeks and detectability for four to six months after stopping. It also covers pregnancy exposure, PSA interpretation and persistent sexual effects.840

01 Does dutasteride work better than finasteride for hair loss?
In the 917-man trial, posted results show adjusted gains of 89.6 hairs with dutasteride versus 56.5 with finasteride at 24 weeks, measured in a 2.54-cm-wide target circle. Dutasteride also performed better on hair width and frontal photographs. These figures describe the studied oral products and men aged 20 to 50.851,839
02 Did the lower-dose trial prove equal results?
The Korean trial included 139 men and treated its higher-dose group as exploratory. The study supports a hair-count benefit over placebo; it was not a formal equivalence trial.852
03 Why did the mood warnings change?
Regulators added precautionary mood warnings after reviewing this drug class. The European review found insufficient evidence for a direct link between dutasteride and suicidal thoughts; UK and Russian notices carry that distinction. The UK notice advises medical contact for depression or suicidal thoughts.849,850,853
04 What do the intermittent-treatment studies show?
A 60-man pilot compared two intermittent dutasteride schedules with daily finasteride. Hair growth improved in all groups. The more frequent dutasteride schedule did not significantly outperform finasteride on hair count over 24 weeks.854
05 Can dutasteride affect sperm?
Yes. The US label reports reductions in sperm count, semen volume and motility in healthy volunteers. Average values remained within normal ranges, while two men had sperm-count reductions greater than 90%. The studies leave individual fertility outcomes uncertain.840
06 What does the newer fertility study add?
A 2025 study followed 200 men attending a fertility clinic after using dutasteride. Some semen measures remained impaired six months after stopping. The study lacked pre-treatment semen results and an untreated group, which limits what it can establish about cause and recovery.848
07 Why does the label warn about leaking capsules?
Dutasteride can pass through skin. The pregnancy warning concerns possible harm to the development of a male fetus, so the label warns pregnant people against handling leaking capsules.840
08 Why is there a blood-donation waiting period?
The US label sets a six-month wait after the last dose to prevent a pregnant transfusion recipient from receiving dutasteride. The drug clears slowly.840
09 Does a lower PSA mean a lower cancer risk?
Dutasteride itself lowers PSA, often by about half within three to six months. Clinicians account for that effect when interpreting results. The label also warns about more high-grade cancers in a prostate-cancer prevention trial.840
10 Is dutasteride approved for hair loss anywhere?
Japan’s Zagallo label includes male-pattern hair loss. The US Avodart label covers an enlarged prostate. Approval depends on the country, product and indication.841,840
11 Does topical dutasteride stay entirely in the scalp?
The 2025 topical trial detected dutasteride in blood. It tested one specific solution and a small pharmacokinetic subgroup. Its exposure table and narrative report different peak values.842
12 How convincing is the topical trial?
It reports improved hair counts over 24 weeks, including a benefit over oral finasteride for the highest-strength solution. The study was small and manufacturer-funded. Participant totals and some safety and blood-level reporting need clarification.842

15 Can sexual side effects persist after stopping?
The US label says sexual effects may persist after stopping, with dutasteride’s role in persistence unknown. The observed sexual events resolved during follow-up in a smaller hair-loss trial. Canada’s June 2026 bulletin describes plans to strengthen sexual-dysfunction warnings.840,857,858
16 Can scalp injections cause hair loss?
Published case series describe bald patches after dutasteride scalp injections, including scarring and incomplete regrowth. How often this happens is not known.843,844
17 Why do studies rank hair-loss treatments differently?
They measure different outcomes over different periods. A treatment can rank first for total hair count at 24 weeks while another leads for terminal hair count. Comparisons also differ in dose, patient selection and whether treatments were tested directly against each other.859
18 Has every trial found dutasteride better than finasteride?
No. Larger trials have favored dutasteride on particular hair measurements, but a 2026 Iranian trial in 46 men found no significant difference in hair improvement after 24 weeks. Trial size and the outcome measured affect the result.851,847
19 What evidence is there for women with hair loss?
A Korean study followed 24 women taking oral dutasteride and found higher hair density, with no increase in average hair thickness. It had no untreated comparison group. The US Avodart label has no indication for women and warns about fetal exposure.860,840
20 What can combination studies tell us?
They show what happened with the whole treatment. A seven-man hair study combined minoxidil, dutasteride and copper peptides while topical minoxidil continued. It used photographs and could not isolate which ingredient contributed.861
21 Is a generic the same as a compounded product?
They follow different approval paths. An FDA-approved generic must meet bioequivalence and other quality requirements. Compounded drugs have no FDA approval review of their safety, effectiveness or quality before marketing.862,863
22 Does a higher price mean it works better?
FDA explains that approved generics can cost less because competition and lower development costs reduce prices, while bioequivalence requirements still apply. Price alone cannot show whether a product is effective.862
23 What does FDA approval tell me about a generic?
FDA reviews generic drugs for quality, manufacturing and bioequivalence. An approved generic must have the same active ingredient, strength, dosage form and route as the reference medicine.862
24 Which schedules have actually been studied?
The 917-man trial studied daily oral dutasteride at 0.02, 0.1 and 0.5 mg for 24 weeks, alongside finasteride and placebo. A later 60-man pilot tested intermittent oral schedules. Each result belongs to its study population, formulation and comparison.851,854
25 Does lowering DHT also lower testosterone?
Dutasteride blocks conversion of testosterone to DHT. In the US-label BPH studies, DHT fell while median testosterone rose, remaining within the physiological range. The two hormone measurements describe different effects.840
26 What does the label say about storage?
The US Avodart label specifies 25°C, with permitted excursions from 15°C to 30°C. It also warns about skin exposure to the capsule contents during pregnancy. Those instructions describe the labeled capsule product.840
27 How long does it stay in the body after stopping?
The US label gives a steady-state half-life of about five weeks. Dutasteride can remain detectable for four to six months after the last dose. The label uses that persistence in its blood-donation precautions.840
28 Why does the label say to swallow capsules whole?
The contents can irritate the mouth and throat. The label says capsules should be swallowed whole rather than chewed or opened. The intact capsule is the formulation covered by those instructions.840
29 Is weight gain a known side effect?
Weight gain is not listed among the common adverse reactions in the US Avodart label. Those tables chiefly describe older men with BPH. They provide limited information about weight changes in younger people treated for hair loss.840
30 Has it been studied for children?
The US Avodart label says pediatric safety and effectiveness have not been established. It has no pediatric indication.840
31 Can other medicines change dutasteride exposure?
Yes. Dutasteride is metabolized by CYP3A4 and CYP3A5 enzymes. The US label calls for caution with long-term use of potent CYP3A4 inhibitors because exposure may increase.840
