Clomiphene

Clomiphene is an oral medicine used in fertility care, with studies of ovulation, testosterone, semen and pregnancy across groups.

Men: hormone and fertility outcomes

Testosterone commonly rises during treatment. Studies also track symptoms, sperm concentration, pregnancy and live birth; the results vary across those outcomes.816,817,818

Approval by country

Approval depends on the country. The US label covers ovulatory dysfunction in women trying to become pregnant. Japan also approves it to induce sperm formation in men with oligospermia.819,820

Long-term follow-up in men

Most male studies are small or retrospective. They report hormone changes and symptoms over months or years, with incomplete information about uncommon harms and children conceived during treatment.822,816,823

Tested sport

The 2026 WADA List names clomifene under S4.2 and prohibits it in and out of competition. The published 2027 List retains that classification.824,825

Common questions

01 Is clomiphene approved for men?

Approval depends on the country. The US label covers ovulatory dysfunction in women trying to become pregnant. Japan also approves it to induce sperm formation in men with oligospermia.819,820

02 Why can a fertility drug raise testosterone?

Clomiphene alters estrogen feedback to the brain, increasing signals that stimulate the testes. In a 153-man clinical series, median testosterone rose from 9 to 16 nmol/L during treatment.820,816

03 Is it as effective as testosterone therapy?

Both can raise testosterone. A 2026 review pooled clomiphene and enclomiphene studies and found no clear testosterone difference compared with gels. Its small libido analysis favored testosterone therapy; direct comparisons of clomiphene itself remain limited.817

04 Does it improve male fertility?

Evidence is mixed. Semen measures improved in several studies, but the placebo-controlled WHO trial of 190 couples found pregnancy in 8.1% with clomiphene and 11.7% with placebo.818,826

05 Can sperm counts get worse?

Yes. In a 2026 retrospective study of 166 men with low testosterone and low sperm concentration, concentration improved in 118 men and declined in 48. Follow-up semen testing tracks that outcome directly.827

06 What happens after stopping?

Testosterone often falls again. In one retrospective study, 48 of 52 men who took a break from treatment had a decrease. The treatment-free interval lasted a median of three months.816

07 Is clomiphene the same as enclomiphene?

Clomiphene contains two geometric forms: enclomiphene and zuclomiphene. Enclomiphene-only products contain one of those forms. Some reviews pool trials of both products, so the results must be read with the drug used in each trial.819,828

08 How did it compare with letrozole in PCOS?

In a randomized trial of 750 women with PCOS, live birth occurred in 27.5% with letrozole and 19.1% with clomiphene over the study treatment course.815

09 Does adding metformin improve the chance of a baby?

In the 626-woman PCOS trial, live birth was 22.5% with clomiphene and 26.8% with the combination; that difference was not statistically significant. A newer Chinese trial reported more clinical pregnancies with the combination and no significant live-birth difference.829,830

10 Which vision symptoms need attention?

Blurred vision, flashes, spots or a loss of vision need prompt medical assessment. The US label and a 2024 safety communication direct patients to stop treatment if unusual visual symptoms appear and have their eyes assessed. Persistent vision loss has been reported.819,821

11 What do longer studies in men show?

A retrospective series included 400 men. Among 120 treated for more than three years, 88% reached the study’s normal testosterone range, 77% reported symptom improvement and 8% reported side effects. The findings describe men who stayed on treatment and were followed in the clinic.822

12 Does it speed recovery after anabolic steroids?

A 79-man observational study reported different semen-recovery rates across treatment groups: 87.5% with clomiphene plus hCG, 69.2% with clomiphene and 58.6% without treatment. Treatment was not randomly assigned, leaving the size of any treatment benefit uncertain.831

13 What does the Russian study add?

The 80-man study reports six conceptions among 30 men receiving clomiphene, followed by live births. It describes randomization between active treatments, while ten men who declined treatment formed the untreated comparison group.832

14 What is known about children conceived during male treatment?

Japan approved the male-infertility indication with a requirement to collect more child-outcome data. Its review considered laboratory genotoxicity findings and limited human follow-up. The label includes counseling about that uncertainty.820,823

15 How long does it stay in the body?

The two forms clear at different rates. The US label reports detectable zuclomiphene for more than a month. A separate six-week finding measured radiolabeled material in excretion, including metabolites; it is not a six-week half-life for the drug.819

16 Why can testosterone go up while I still feel bad?

Blood tests and symptoms can move differently. In the placebo-controlled obesity study, a later review reports that overall symptom scores improved in both groups without a clear difference between them. A separate small comparison found lower libido scores among men taking clomiphene.828,833

17 How long before testosterone or sperm counts change?

One follow-up study found higher testosterone at three months, with further improvement at six months. Sperm concentration first improved at nine months. Those analyses included only 25 men with complete hormone follow-up and 26 with complete semen follow-up.834

18 Why do people respond differently?

Starting hormone levels, the cause of low testosterone and drug metabolism can all affect the result. A single-dose study in 22 healthy Korean people found different active-metabolite exposure across CYP2D6 gene variants. A separate clinical cohort found weaker testosterone responses in men with higher starting LH.835,836

19 Does it help with weight loss or building muscle?

None of the cited studies establishes a muscle-building benefit in healthy men. A review of clomiphene and enclomiphene trials in men with low testosterone found no clear BMI benefit over placebo. Research in men with obesity and low testosterone has measured body composition separately from athletic performance.828,837

20 Does adding hCG make it work better?

In an active-comparator trial, testosterone rose with clomiphene, hCG and their combination. The combination improved the symptom questionnaire more than either treatment alone, while its testosterone increase was not clearly greater than clomiphene alone. The trial had no placebo group.838,828
