AZD6234

updated

AZD6234 is AstraZeneca's weekly injectable amylin receptor agonist, which the company calls selective. The only public human efficacy figure is second-hand: a 2026 review of a 2024 conference abstract gives the greatest mean weight loss as 3.8% from baseline by day 5 after a single dose in healthy participants. Three phase 2 studies are past primary completion without posted results, and three phase 3 trials started in August 2026.

Amylin receptor agonist (weekly injectable; AstraZeneca calls it selective)

  • AstraZeneca's Form 6-K of 30 January 2026 describes AZD6234 as a weekly injectable selective amylin receptor agonist (SARA). No FDA application or label was found on 10 October 2026, and PubChem has no record.
  • Three AstraZeneca phase 3 trials (SELENE 1, 2 and 3) have start dates from 19 to 24 August 2026, a primary outcome of percent change in body weight at 68 weeks, and no results.
  • Three phase 2 studies (NCT06595238, NCT06851858, NCT06862791) reached primary completion between December 2025 and May 2026, and none has posted results.
  • A 2026 review citing a 2024 conference abstract says the greatest mean weight loss was 3.8% by day 5 after a single 2.7 mg subcutaneous dose, with a terminal half-life of about 4 days. These figures are second-hand.
  • In diet-induced obese rats, AZD6234 at 7.5 nmol/kg daily for 28 days gave 9.3% weight loss versus 9.9% for semaglutide at 5 nmol/kg and 19.1% for the combination (AstraZeneca conference abstract).
  • AstraZeneca-originated records link the name lenbelintide to AZD6234, but the link is not confirmed, and WHO calls lenbelintide a dual amylin/calcitonin receptor agonist.

What is AZD6234, and is it approved?

AZD6234 is AstraZeneca's weekly injectable amylin receptor agonist, which the company calls selective. It has no approval record: FDA database searches on 10 October 2026 found no application or label, and no structure is public.

AstraZeneca's Form 6-K of 30 January 2026 describes AZD6234 as "a weekly injectable selective amylin receptor agonist (SARA)" in its weight management portfolio, next to the oral GLP-1 agonist elecoglipron and the GLP-1/glucagon agonist AZD9550. AstraZeneca's 2025 annual report names AZD6234 phase IIb trials in obesity (APRICUS) and type 2 diabetes (ARAY) and a combination trial with AZD9550 (ASCEND). ClinicalTrials.gov lists AstraZeneca as sponsor of every AZD6234 trial found.

PubChem returned no record for AZD6234 on 10 October 2026, so no structure, formula or molecular weight is public in the records reviewed.

openFDA Drugs@FDA queries for AZD6234 returned "No matches found" on 10 October 2026, while a control query for semaglutide returned records, and a DailyMed drug name search returned 0 labels. The same was true for the name lenbelintide, which appears in AstraZeneca-originated records as a possible name for AZD6234 and is discussed below as a link that is not confirmed. Status outside the United States is not covered here.

What human results exist for AZD6234?

Very little is public. The only human efficacy figure is second-hand: a 2026 review of a 2024 conference abstract gives the greatest mean weight loss as 3.8% from baseline by day 5 after a single dose. Three phase 2 studies are past primary completion without posted results.

NCT05511025 (AstraZeneca) is a randomized, single-blind, placebo-controlled single ascending dose study in healthy participants with overweight or obesity aged 18 to 55, 54 participants (actual), completed on 19 December 2023, with no posted results. A 2026 narrative review (PMID 42452898), citing a 2024 conference abstract by Rauschecker and colleagues that was not retrieved, says AZD6234 showed dose-proportional pharmacokinetics and a terminal half-life of about 4 days, and that the greatest mean weight loss was 3.8% from baseline by day 5 after a single 2.7 mg subcutaneous dose. Nausea and vomiting rose with exposure, and the review's table lists nausea 32.1% and vomiting 17.9% without a denominator. These figures are second-hand and could not be confirmed against the abstract.

Phase 2 studies are complete or past primary completion, and none has posted results. NCT06595238 (AstraZeneca, 262 participants actual, once-weekly injection for 36 weeks, primary outcomes percent weight change and at least 5% weight loss at week 26, primary completion 3 December 2025), NCT06851858 (type 2 diabetes on a stable GLP-1 agonist, 69, week 26 outcomes, primary completion 26 May 2026) and NCT06862791 (AZD6234 with AZD9550, 377, week 36 outcome, primary completion 11 May 2026). The annual report's trial names (APRICUS, ARAY, ASCEND) are not in the registry titles, so their match to these NCT numbers rests on design only. No AstraZeneca statement of phase 2 weight-loss figures was found. A registration is not an outcome.

Other phase 1 studies are registered with no posted results: NCT06132841 (repeat dose in participants with overweight or obesity, single-blind, 104 participants, completed, last updated 3 April 2026), NCT06845813 (renal impairment, 24, completed), NCT07220954 (subcutaneous formulations, 19, active not recruiting), NCT07013643 (oral contraceptive interaction with AZD6234 and AZD9550, 50, recruiting) and NCT07546760 (hepatic impairment, 28, recruiting). No repeat-dose, renal or interaction results are public in the records reviewed.

PubMed queries on 10 October 2026 for AZD6234, AZD-6234 and lenbelintide returned 2, 0 and 0 records. The two records are 2026 narrative reviews (PMIDs 41747885 and 42452898) that list AZD6234 among amylin analogs. Their abstracts give no AZD6234 trial data, one abstract says several analogs including AZD6234 have shown efficacy as monotherapy in clinical trials, and the full text of 41747885 was not read. No peer-reviewed report of AZD6234 trial data was found.

Has phase 3 started?

Yes. Three AstraZeneca-sponsored phase 3 trials (SELENE 1, 2 and 3) are recruiting, with start dates from 19 to 24 August 2026, a primary outcome of percent change in body weight at 68 weeks and no results. A registration is not an outcome.

SELENE 1 (NCT07784725) enrolls adults with obesity or overweight and a comorbidity without type 2 diabetes (estimated 2500, start 19 August 2026), SELENE 2 (NCT07784270) adults with type 2 diabetes and obesity or overweight (estimated 1500, start 20 August 2026) and SELENE 3 (NCT07776509) an add-on to incretin therapy (estimated 500, start 24 August 2026). All three are recruiting with no results, and the primary outcome is percent change in body weight at 68 weeks. What do the rat data show?

In diet-induced obese rats, AZD6234 at 7.5 nmol/kg daily for 28 days gave 9.3% body weight loss versus 9.9% for semaglutide at 5 nmol/kg and 19.1% for the combination, per an AstraZeneca conference abstract. This is rat data, not evidence in people.

A Diabetes 2025 supplement conference abstract (88-OR, published 20 June 2025, AstraZeneca authors) gave male diet-induced obese Sprague Dawley rats daily subcutaneous doses for 28 days. AZD6234 (7.5 nmol/kg) and semaglutide (5 nmol/kg) produced similar body weight loss versus vehicle (9.3% and 9.9%), and the combination produced 19.1%. Food intake fell by about 20% with each alone and 41% combined.

AZD6234 lost more fat mass than semaglutide (17.7 g versus 11.4 g) while lean mass was preserved, but the combination lost lean mass (4.7 g). This is rat data from a conference abstract, not a full peer-reviewed paper, and it is not evidence in people.

Is AZD6234 a selective amylin receptor agonist in phase 2?

Half right. AstraZeneca's own filing calls AZD6234 a weekly injectable selective amylin receptor agonist, but the phase 2 label is out of date: three phase 3 trials started in August 2026.

The claim describes AstraZeneca's AZD6234 as a selective amylin receptor agonist said to be in phase 2.

The selective amylin receptor agonist description is confirmed in AstraZeneca's Form 6-K of 30 January 2026. Phase 2 is out of date: three phase 2 studies reached primary completion between December 2025 and May 2026 with no results posted, and AstraZeneca registered three phase 3 trials with start dates from 19 to 24 August 2026.

Selectivity is AstraZeneca's own description. The WHO descriptor for lenbelintide, a name that AstraZeneca-originated records link to AZD6234 but that is not confirmed as the same compound, is dual amylin/calcitonin receptor agonist, and that conflict is unresolved. No AstraZeneca statement of phase 2 weight-loss figures was found.

2026 narrative review: second-hand AZD6234 phase 1 figures from a 2024 conference abstract

Narrative review

Alhazmi A et al. Diabetes Obes Metab. 2026;28 Suppl 5(Suppl 5):42-50. PMID 42452898.

A 2026 narrative review, citing a 2024 conference abstract by Rauschecker and colleagues that was not retrieved here, says AZD6234 showed dose-proportional pharmacokinetics and a terminal half-life of about 4 days, and that the greatest mean weight loss was 3.8% from baseline by day 5 after a single 2.7 mg subcutaneous dose. Nausea and vomiting rose with exposure, and the review's table lists nausea 32.1% and vomiting 17.9% without a denominator.

What risks have been reported?

No AZD6234 safety table is public. A 2026 review citing a 2024 conference abstract lists nausea 32.1% and vomiting 17.9% after a single dose, without a denominator, and the registered repeat-dose, renal, hepatic and interaction studies have no posted results.

The only reported adverse-event figures are second-hand. A 2026 narrative review (PMID 42452898), citing a 2024 conference abstract that was not retrieved, says nausea and vomiting rose with exposure after a single dose, and its table lists nausea 32.1% and vomiting 17.9% without a denominator. These figures could not be confirmed against the abstract.

Repeat-dose (NCT06132841, 104 participants), renal impairment, subcutaneous formulation, oral contraceptive interaction and hepatic impairment studies are registered with no posted results. No phase 2 or phase 3 result exists, so tolerability with repeated weekly dosing is not described in any public source reviewed.

In rats, the AZD6234 plus semaglutide combination lost lean mass (4.7 g) while AZD6234 alone preserved it. That is a conference abstract in rats.

Conference abstract, rats: AZD6234 9.3% and semaglutide 9.9% weight loss, 19.1% combined

Animal study

Diabetes 2025 supplement, abstract 88-OR, published 20 June 2025, AstraZeneca authors. DOI 10.2337/db25-88-or.

In male Sprague Dawley diet-induced obese rats given daily subcutaneous doses for 28 days, AZD6234 (7.5 nmol/kg) and semaglutide (5 nmol/kg) produced similar body weight loss versus vehicle (9.3% and 9.9%), and the combination produced 19.1%. Food intake fell by about 20% with each alone and 41% combined. AZD6234 lost more fat mass than semaglutide (17.7 g versus 11.4 g) while lean mass was preserved, but the combination lost lean mass (4.7 g).

Why does AZD6234 sit in C tier?

The only public human efficacy figure is a single-dose weight change taken second-hand from a conference abstract review, plus rat data. Phase 2 and phase 3 studies are registered, but no phase 2 result has been posted or published. The data are thin rather than negative.

The C reflects a single-dose weight change in healthy participants, taken second-hand from a review of a 2024 conference abstract, plus rat data from an AstraZeneca conference abstract. Three phase 2 studies reached primary completion between December 2025 and May 2026 and AstraZeneca registered three phase 3 trials in August 2026, but no phase 2 result has been posted or published, and a registration is not an outcome.

The amylin mechanism has support from class data, such as the published phase 2 trial of petrelintide on its own page, so the data on AZD6234 are thin rather than negative. The grade sits below the B given to cagrilintide and eloralintide, which have published trial data.

Why is AZD6234 in C tier?

C fits a compound whose only public human efficacy figure is a single-dose weight change in healthy participants, taken second-hand from a review of a 2024 conference abstract, alongside rat data from an AstraZeneca conference abstract. Three phase 2 studies reached primary completion between December 2025 and May 2026 and three phase 3 trials were registered in August 2026, but no phase 2 result has been posted or published.
